Dear Patient or Patient’s Legal Representative,
It is with a heavy heart that I am writing to announce my plans to retire from the practice of medicine effective May 31, 2023. It has been a privilege to take care of you at Patel Medical Clinic. I am so appreciative of the trust you have placed in me as your physician and regret not being able to be part of your future health needs.
Regarding your future care, you are free to choose any provider of your choice, but I believe the practices below will provide you with the excellent care that you deserve.
For Internal Medicine (Primary Care):
For your convenience and to maintain the continuity of your primary care, I have made arrangements with Piedmont Family Practice as well as Fort Mill Family Practice to provide your future primary care. I have listed their information below and you are welcome to contact these practices to schedule an appointment.
For Cardiology:
For your convenience and to maintain the continuity of your heart care, I have made arrangements with Carolina Cardiology Associates (CCA) to provide your future care. CCA is the premier cardiology practice in the area, offering convenient offices in Rock Hill and a newly-built facility in Fort Mill Medical Professional Building located on the new Fort Mill Hospital campus which is only 5 minutes from my office. I am confident CCA will provide you with excellent care for all your heart and vascular needs due to their experienced caring physicians and state-of-the-art facilities. An agent from CCA will be contacting you prior to my closure to schedule your next cardiology appointment at one of their offices.
The last day that I will see patients will be May 31, 2023. I will continue to provide medical care, including emergency care, for you prior to this closing date. After May 31, 2023, the office will be closed, and further care including prescription refills will be provided by your new physicians.
Custodianship of your medical records has already been transferred to Carolina Cardiology Associates and they will continue to maintain your records for 10 years from the date of last service. You or your Primary Care Provider can contact and request your full medical records from CCA as follows:
